All Tacoma students, grade K-12 interested in participating in intermural, recreational or school sports are invited to attend this FREE Sports Physical and Health Fair, Thursday, August 16 from 3 to 5:30 p.m.

Lincoln High School

Baker Middle School
8001 South J Street, Tacoma

FREE
-> Sports Physicals (Distance eye screening, height/weight, blood pressures, and physical exam provided by doctors.)
-> Immunizations (*See below)

Students under 18 must be accompanied by a parent or guardian. Paperwork can be completed upon arrival.

*Sixth grade and pre-college vaccines will be available at no cost to parents for all students. The vaccines recommended for 11-12-year olds are TDaP (tetanus and whooping cough prevention), HPV9 (cancer prevention), and MCV4 (meningitis prevention). Pre-college vaccines include a second dose of MCV4 and HPV9 (if still needed). Influenza vaccine will be offered if available. We will be able to look up your child’s record and tell you if they are school ready. If you have questions, please Call Carolyn Cook RN at 253-798-4428.
